Maine Notarial Certificate – Jurat for Affidavit

The Maine jurat affidavit certificate is almost identical to the Maine Jurat Notarial Certificate, however, it includes the actual language for an affidavit. The affidavit is included at the top of the form and must be signed by the individual making the oath/affirmation; this section must be signed in front of a notary public. The notary will then administer an oath or affirmation before completing the notarial certificate.The notary is recommended, but not obliged, to seal the notary acknowledgement form.
