A Tennessee Affidavit is a sworn statement by an affiant in Tennessee which is then notarized by a jurat. The affiant (individual making statement and swearing to its validity) must present themselves before the notary, make their statement, and swear (or affirm) under penalty of perjury that the statement is true. They must also sign the document on the appropriate field. The notary will then complete the jurat and administer their seal.
