A Maryland short form oath or affirmation certificate is reserved for legal instruments wherein an oath or affirmation statement has been included in the language of the document. The document in question shall be brought to a notary public, who will complete the same notarial duties required of a Maryland Notary Acknowledgement Form – Oath or Affirmation. Although a statement of oath or affirmation is contained within the document, each signatory must still make a verbal oath/affirmation to the notary, and shall sign the document in the notary’s presence.
