Maryland Notary Acknowledgement – Representative

A Maryland representative notary acknowledgement form is used when a document signed by a public officer, trustee, administrator, guardian, or executor requires notarization. The document must have the acknowledgement form included therein, or attached thereto, before being presented to a notary public. The notary will identify the signer(s), either personally or by proper identification, and will have them make the necessary acknowledgements. Lastly, the notary will complete the acknowledgement form and include their seal and signature at the bottom.
