The South Carolina Affidavit form is used to record and notarize a statement made by the affiant. The individual in question must appear before the notary, take an oath (vow of truthfulness invoking a deity) or an affirmation (vow of truthfulness based on personal honor), and complete the affidavit form by writing their statement(s). Once this has been completed, the notary public will fill out the remainder of the form, sign it, and administer their seal. These documents are used most commonly during court proceedings and in other legal matters.
