Kentucky Notary Acknowledgement – Individual

The Kentucky individual notary acknowledgement form is used for notarizing a document where an individual has signed in his/her own right. The instrument requiring notarization should be presented to a notary public with the acknowledgement form attached to or included at the end of the instrument. The notary will ask that the signer(s) show proof of identification, unless the notary knows them personally. Next, each signatory will acknowledge that they signed the document under their own will. Once the notary has completed these duties, they will fill out the notary acknowledgement form.
