Illinois Notarial Certificate – Verification Upon Oath or Affirmation by Representative

The Illinois verification upon oath or affirmation by representative provides proof that an individual attested to the truthfulness of a document they signed on behalf of another person or entity. A notary public will need to check the identity of the signer and ensure they are permitted to act on behalf of the principal. Following confirmation of the identity, the notary will ask the representative to sign the document and say an oath (or affirmation) verifying the information contained therein. To finish the notarial process, the verification form will be filled out, signed, and sealed by the notary.
