The Wisconsin Notarial Certificate for an Oath or Affirmation is a document used to verify that an oath or affirmation took place validating the truthfulness of a legal document or statement. The notary public will ask the individual to swear before a deity (take an oath) or affirm (on their honor) that the statement is true to the extent of their knowledge. The individual must then sign the document or statement in the presence of the notary, after which point the notary will complete the notarial certificate and finalize the process.
