The Maryland notary oath or affirmation form is a type of notarial certificate that does not technically require an acknowledgement, but demands that an oath or affirmation be made by the signing party(ies) attesting to the truthful of the contents of a particular document. The signer(s) must attach this certificate to the instrument requiring notarization and present it to a notary public (identification will most likely be required). Each signer will swear an oath (solemn statement to Divine Being) or affirmation (statement without appeal to a Divine Being) before the notary. For this type of certificate to be valid, the signing party(ies) must sign the document in the notary’s presence. The notary will sign and seal the certificate once these duties have been accomplished.
