The Nevada notarial certificate for administering oath or affirmation of office is used when a notary public is administering an oath or affirmation of office. An oath is when an individual swears on the name of their deity/godhead and an affirmation is a claim of honesty; both forms are made by penalty of perjury. The individual making the oath or affirmation will fill out the statement, provide identification to the notary, and say aloud the statement, as written, in their presence. The oath-taker will then sign the document, at which point the notary may certify it.
