The Nevada individual notary acknowledgement form is used by individuals acting on their own behalf who need to have a document notarized. The individual must bring the completed and unnotarized document (with the below form attached), along with at least one piece of photo ID, to a notary public. Once the notary has verified the signer’s identity and signature, they can fill out the notary acknowledgement form. The individual does not need to sign their document in front of the notary.
