California Notary Acknowledgement Forms
The California notary acknowledgement forms are certificates verifying that a notary public has confirmed the identity of a person who signed a legal document. An individual seeking acknowledgement must appear before the notary public and present satisfactory identification (e.g., passport, driver’s license). If no personal identification documents are available, they may prove their identity through the oath or affirmation of a credible witness known to the notary or two (2) credible witnesses not known to the notary. In either case, the notary must be certain the signing party has executed the document for the reasons outlined in the acknowledgement statement.
