This Massachusetts notary acknowledgement form is applicable to all documents signed by any type of signatory; partner, corporation, attorney in fact, or any other person or representative. This notarial certificate must be included in or attached to the legal instrument that needs notarizing (deed, trust agreement, power of attorney, etc.). The signing party must present the document to a notary public (signature can already be inscribed) and show identification. An acknowledgement must be made by the signer declaring that they signed the document under their own free will.
