Washington Verification Upon Oath or Affirmation

The Washington Verification Upon Oath or Affirmation is a form used to validate a legal document or affidavit. The language “signed and sworn to (or affirmed before me)” indicates that the signer of a legal document presented themselves before a notary public and took an oath (sworn before God) or an affirmation (sworn on their honor) that the information in the legal document is 100% accurate. They will then need to sign the document in the officer’s presence. The notary will complete the short form and provide their seal thus notarizing the affidavit or legal instrument.
