The Alaska short form notary acknowledgement for an individual is a certificate indicating that a notary public has confirmed that an individual has signed a document and that the person gave their signature willfully. A notary public is not required to witness the signing in order to confirm the signature, though the individual must personally appear before the notary and identify himself/herself as the person qualified to execute the document attached to or included with the written acknowledgement form.
