An Oregon notary acknowledgement for an individual acting in a representative capacity is used by notary publics when authenticating the signature of someone who has signed a legal instrument on behalf of a third party. This document must be filled out, signed, and stamped by a registered notary public. The representative will need to bring the signed copy of the instrument to a notary and identify themselves, providing their title or type of authority and the name of the individual or entity (i.e., corporation, LLC) that they are representing. If everything appears to be in order, the notary will fill out the notary acknowledgement form. State law requires that the notary must affix their official stamp to the form in order for it to be validated.
