The Florida notarial certificate (oath or affirmation signed by mark) is an instrument that a notary public will use when an individual, whose signature is a mark, attests to the accuracy of information in a document. Also known as a “jurat,” this certificate states that the individual appeared before the notary, presented a sufficient form of identification, and swore to (or affirmed) the truthfulness of the document requiring notarization. The individual must sign the document (provide their mark) in the presence of the notary and two (2) disinterested witnesses. After the mark is made, both witnesses must write their names, addresses, and signature on the notarial certificate.
On the “His (or Her) Mark” signature field of the notarial certificate, the notary must write the individual’s first name on the far left followed by the last name on the far right. The space between the first and last names is designated for the individual’s mark.
